Granite Orientation Informed Consent


The purpose of a physical fitness program is to learn the proper use and form for physical activity, and to design a balanced program which fits the needs of you, the participating member as closely as possible so as to maintain and/or improve your physical condition.

You understand that you are responsible for monitoring your own condition throughout your workouts. Should any unusual symptoms occur, you will cease your participation and inform your trainer of the symptoms.

In the event that a medical clearance must be obtained prior to your participation in a workout program, you agree to consult your physician and obtain written permission from their physician prior to commencing any physical workouts with a trainer.

You acknowledge that you have read this form in its entirety, that all questions have been answered correctly, and that all of your questions have been answered to your satisfaction. You further agree to assume the risk of any physical activity while working with a trainer or in a Granite Health & Fitness location in general, and also agree to hold harmless all trainers, management, instructors and/or staff of Granite Health & Fitness, Inc., from any and all claims, suits, losses or related causes of action for damages, including but nor limited to, such claims or damages that may result or arise from injury or death related in any way from utilizing a personal trainer.
